PLEASE READ THESE INSTRUCTIONS BEFORE PROCEEDING:

When you’re returning to the site AFTER registering, you can select any option depending on what you need. For your FIRST time, please use the APPOINTMENT website's top left box. (Please REGISTER ONLY ONCE....after your initial resgitration, use the other tabs if you need to modify your information or schedule an appointment.)  Complete as much of the information as you want and then schedule your next appointment. If you want to reserve repeating appointments, you may do that, too. Insurance: You may optionally enter your insurance information. I use a fee scale based on your household income. I will also provide your with a superbill that you may optionally file with your insurance company for any benefits that you have covered in your health insurance contract. Payment is due when we meet, unless we have made prior arrangements.
